March 2026 Event Waiver

WAIVER OF LIABILITY, INFORMED CONSENT, AND RELEASE AGREEMENT

Event Date: March 28, 2026

Event Location: Capital Bluffs Event Center, 1616 Oilwell Rd Suite B, Jefferson City, MO 65101

This Waiver of Liability, Informed Consent, and Release Agreement (“Agreement”) is entered

into by the undersigned participant (“Participant”) in connection with participation in an event

hosted by Jen’s Get Fit Group, LLC (“JGFG”), Jennifer Loganbill, BOMI WELLNESS CO.

(“BOMI”), Fit Factory Supplements, and any associated entities, representatives, or affiliates

(collectively, “Event Hosts”).

Acknowledgment of Risk

I, the undersigned Participant, voluntarily choose to participate in the Event and acknowledge

that participation may involve certain inherent risks, including but not limited to physical

injury, property damage, illness, or even death. I fully assume all risks associated with my

participation in the Event, including risks arising from the Event Hosts’ negligence or that of

any third parties.

I confirm that I am physically capable of participating in the Event and have not been advised

by a medical professional to refrain from physical activities. If I experience any health

concerns, limitations, or injuries during the Event, I agree to immediately inform the Event

Hosts.

Release of Liability

In consideration of being allowed to participate in the Event, I hereby agree to waive, release,

and forever discharge the Event Hosts, their officers, members, employees, agents,

representatives, affiliates, successors, and assigns from any and all claims, demands, liabilities,

or causes of action, whether known or unknown, arising out of or in connection with my

participation in the Event. This includes but is not limited to claims for personal injury, illness,

property damage, death, or any other loss, even if arising from the negligence of the Event

Hosts or others.

I further agree to indemnify, defend, and hold harmless the Event Hosts from any and all

claims, costs, damages, or expenses, including attorney fees and legal costs, that may arise

directly or indirectly from my participation in the Event or from any breach of this Agreement.

Merchandise Sales Policy

I acknowledge and agree that all merchandise purchased at the Event is final sale. No

exchanges, returns, or refunds will be permitted under any circumstances.

Severability and Binding Effect

This Agreement shall be binding upon me, my heirs, legal representatives, successors, and

assigns. If any provision of this Agreement is found to be unenforceable or invalid, all other

provisions shall remain fully enforceable.

Medical Information Release

I consent to the release of my name and any medical information related to any injury, illness,

or death I may suffer during the Event to the Event Hosts and their insurance providers as

necessary.Modification and Waiver

This Agreement may not be modified except in writing signed by the Participant and a duly

authorized representative of the Event Hosts. Any waiver of a specific provision shall not be

construed as a waiver of any other provision or subsequent waiver of the same provision.

Voluntary Participation and Understanding

I acknowledge that I have carefully read and fully understand this Agreement. I voluntarily

agree to its terms and conditions, recognizing that by signing this document, I am waiving

certain legal rights, including the right to sue the Event Hosts for any injuries or damages I

may sustain.
